The 2023-2027 Douglas County Community Health Improvement Plan (CHIP) represents a strategic, long-term effort for communities and organizations across Region G to address the primary health priorities identified in the 2021 Region G Community Health Assessment (CHA). Considering the influence of the social determinants of health, the various elements that may elevate health risks among specific populations, and the imperative of achieving health equity, multiple organizations collaborated under the guidance of the Mobilizing for Action through Planning and Partnerships (MAPP) 2.0 framework. This community-based approach engaged stakeholders at all levels to conduct the CHA and develop the CHIP.
