The Douglas County Health Department is the Local Registrar for all births and deaths that occur in Douglas County and can issue certified copies of birth certificates for anyone born in the State of Missouri after 1920. We can issue certified copies of death certificates for anyone dying in the State of Missouri after 1980. Certified copies of other years of birth or death can be obtained from the Missouri Department of Health and Senior Services.

Required items are the Application (which can be printed from above link or they are provided at the Health Department) and Proper Identification (driver’s license is preferred). The certificate will be printed while you are in the department.  We accept cash and debit/credit.  If you use a card a two dollar fee will be added. In-Person Requests:

The Douglas County Health Department can provide birth or death certificates for the State of Missouri between 7:30 and 5:30 Monday thru Thursday.  No appointment is necessary. 

Mail-In Requests:

Requests for records in the State of Missouri must be NOTARIZED by an acceptable notary public and mailed with a MONEY ORDER and SELF-ADDRESSED STAMPED RETURN ENVELOPE.
